What Youll Do as a Spiritual Care Coordinator:
Provide compassionate individualized spiritual care to patients and families honoring their values beliefs and cultural backgrounds
Conduct spiritual assessments and create tailored care plans as part of the interdisciplinary hospice team
Offer emotional and spiritual counseling facilitating meaningful conversations around faith forgiveness and life closure
Participate in team meetings to ensure holistic care that respects every patients spiritual needs
Document services and maintain compliance with all standards and regulations
Support families throughout the hospice journey including bereavement care
Educate staff and families on the role of spiritual care within hospice services
Advocate for patients spiritual needs ensuring their beliefs are honored in all aspects of care
Promote and embody Elara Carings mission vision and values in all interactions
Ensure patient and staff confidentiality in compliance with HIPAA
Perform additional duties or projects as assigned
What Youll Bring:
Bachelors or Masters degree in Divinity Theology Pastoral Counseling or a related field
Ordination or certification as a chaplain clergy or spiritual counselor
At least 2 years of experience in hospice care pastoral ministry or counseling
Strong interpersonal skills and a deep sense of empathy and integrity
Willingness to travel 50 of the time)
Ability to lift up to 50 lbs and perform physical duties as needed
Valid drivers license insurance and reliable transportation
