What you bring to SBHG:
Masters degree in psychology social work child care counseling or development or other related field.
Masters degree in social work.
2 years direct service experience with severely emotionally disturbed adolescents in an inpatient health care setting preferred.
Licensed as LCSW required.
How you will make a difference:
- Assure all legal admission requirements are obtained for each new client. Involuntary 5150 voluntary by youth (Roger S) conservatorship or clients signed in by parents/legal guardians.
- Assure LPS Holds are completed daily (must be LPS Certified).
- Work closely with social worker staff to assure that daily duties were completed including hearings.
- Review treatment plans for interventions and effectiveness.
- Provide consultation and support as needed regarding discharge planning.
- Assure discharges are in compliance with title 22 regulations and families are provided a warm hand off to their follow up services.
- Provide training and consultation regarding treatment of clients discharge planning case management conservatorships community placements etc. as needed.
- In collaboration with the Clinical Director ensure that the PHF is in compliance with all Federal State and local regulations pertaining to clinical mental health services and Patients Rights
- Provide consultation as needed with families on issues regarding admission discharge and aftercare plans.
- Communicate to clients their legal status and rights under applicable law and acknowledge in writing their understanding of same.
- Maintain a knowledge of community resources.
- Coordinate client intake treatment and discharge actions with county partners.

- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare

In accordance with California law the salary grade for this position is 26.16 41.85. Starting pay within the grade is determined based on experience internal equity and other factors permitted by law.
