The Grand Rehabilitation and Nursing Center at Guilderland is seeking a Temporary Social Worker
Location: Altamont NY
Position Type: Temporary 14Week Assignment (Starting June)
Pay Rate: $17.00 to $19.00 per hour

Position Overview
The Grand Rehabilitation and Nursing Center at Guilderland is seeking a Temporary Social Worker for a 14week assignment starting in June. In this role youll support residents and their families through counseling care planning and coordination with our healthcare team.
Key Responsibilities
Provide emotional support and counseling to residents and families
Assist with care planning and discharge coordination
Connect residents with community resources
Collaborate with staff to ensure comprehensive resident care
Qualifications
Bachelors degree in Social Work or related field
Previous experience in a healthcare setting preferred
Strong communication and interpersonal skills
Compassionate and organized approach to care
