Job Summary:
This position is a community clinic-based case management role focusing on initial and 180-day assessments for program participants. The Case Manager will collaborate with hospital discharge planners to ensure safe and appropriate discharge plans coordinate skilled nursing facility (SNF) discharges and work within an interdisciplinary team (IDT) to maintain continuity of care. Experience in case management is required.

Responsibilities:
- Conduct initial and 180-day assessments for assigned participants.
- Collaborate with hospital discharge planners to facilitate safe and appropriate discharges.
- Coordinate SNF discharges for participants requiring rehabilitation or skilled care.
- Work within an interdisciplinary team (IDT) to ensure continuity of care.
- Participate in hospital discharges communicating recommendations from the Providence PACE IDT.
Required Skills & Certifications:
- Minimum of one year of case management experience.
- Masters degree in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- Basic Life Support (BLS) certification.
Preferred Skills & Certifications:
- Licensed Clinical Social Worker (LCSW).
Special Considerations:
- Experience working with geriatric or chronically ill populations is beneficial.
- Remote work options may be available; confirm with the hiring manager.
- No weekend or on-call requirements.
- First-time travelers are not accepted; experience as a traveler is preferred.
Scheduling:
- Five 8-hour days per week.
