Job Description

SUMMARY: Provides case management and community psychiatric support treatment in his/her assigned cottage as a member of the residents clinical team. Clinical Case Manager is an integral part of the cottage staff and must work well with both the clinical and operational teams to support the therapeutic recommendations established by the clinical therapist.


Clinical Case Managers communicate with residents community team including but not limited to juvenile courts child welfare public school systems family and children first councils attorneys doctors and other youthservice persons/agencies. This communication includes coordination and linking services speaking publicly problemsolving and working with family members. Clinical Case Manager position is the bridge between the residents treatment experience at CCHO and the clients community team which includes coordinating stepdown plans upon discharge and continuing care coordination for each client. Clinical Case Manager works as a team with clinical and operational teams in an effort to best meet the needs of the residents.


All Agency staff are expected to focus on Quality improvement as a part of their job responsibilities in an effort to make the Agency a safe and healing environment for clients and staff. This expectation includes but is not limited to: teamwork improving service delivery fulfilling department goals and other jobspecific tasks that encourage excellence.


ESSENTIAL DUTIES and RESPONSIBILITIES: It is the responsibility of each employee to be a consistent witness for Jesus Christ and to adhere to the Standard of Moral Conduct and Statement of Faith. Jobspecific essential functions include the following:

  • Makes every effort to maintain a team first spirit and atmosphere within the workplace environment fostering and maintaining appropriate working relationships within the Childrens Residential Center team and with other Agency departments as well.
  • Communicates effectively with the cottage team both verbally and in writing regarding treatment progress planning decisions problems etc.
  • Completes appropriate logs reports and forms including all billable notes (includes checking alerts consistently to reconcile them) nonbillable case notes and individualized treatment plan (ITP) reviews within agency deadlines and standards.
  • Completes/submits monthly progress reports according to agency standards & deadlines.
  • Completes Campus Champion reports monthly and submits to Advancement team.
  • Ensures that any necessary scales are completed including Casey Life Skills (clients age 14 and Ohio Scales for each client according to agency standards and deadlines.
  • Facilitates group sessions assisting individuals to find hope and insight into their present and future dreams/goals.
  • Meets with each resident on an individual basis regarding continuing care/case needs. Meets weekly productivity requirements according to agency standards.
  • Attends meetings and shares observations about resident issues regarding continuing care (court dates placement reviews county meetings community service activities family phone calls family time (visitation) school progress employment opportunities family therapy needs psychiatric supports etc..
  • Attends ITP Reviews Semi Annual Reviews court hearings and other client community appointments that require therapeutic support and communication with clients community team. Works to prepare client for these appointments.
  • Assists with CRC intake team and clinical team to coordinate new residents transitions to cottage life including but not limited to securing clothing voucher/other needed supplies working with Summit Academy to ensure transition to CCHOs educational setting.
  • Works with the clinical therapist to coordinate after care.
  • Updates and obtains releases as needed to ensure confidentiality.
  • Updates/communicates fact sheet changes to ensure information about approved contacts telephone numbers etc. is accurate.
  • Manages cottage calendar to ensure that client appointments and sessions are up to date and accurate. Coordinates and schedules all client phone calls and family time. Supervises family time/phone calls as needed.
  • Earns and maintains Safe Crisis Management certification and actively engage with clients using SCM skills when needed.

QUALIFICATIONS: To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required. EDUCATION and/or EXPERIENCE: Bachelors degree from a fouryear college or university in social work or a related field plus 12 years related experience and/or training; or equivalent combination of education and experience.

LANGUAGE SKILLS: Excellent verbal and written communication skills required. Ability to read analyze and interpret general business periodicals professional journals technical procedure or governmental regulations. Ability to write reports business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of manages clients customers and the general public.

MATHEMATICAL SKILLS: Ability to add and subtract multiply and divide in all units of measure using whole numbers common fractions and decimals. Ability to compute rate ratio and percent and to draw and interpret bar graphs.

REASONING ABILITY: Ability to define problems collect data establish facts and draw valid conclusions. Ability to interpret and extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

COMPUTER SKILLS: To perform this job successfully an individual should have knowledge of database software and design software. Ability to use a computer email etc.

CERTIFICATES LICENSES REGISTRATIONS: Licensed Social Worker (or licenseeligible) required. Valid Ohio Drivers license must be eligible for CCHO automobile insurance.

OTHER SKILLS & ABILITIES: Strong teamwork and organization/time management skills strongly preferred.

PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

While performing the duties of the job the employee is frequently required to walk sit talk and hear including requirement to stand or walk for extended periods of time often through uneven terrain. The employee is occasionally required to stand use hands/fingers handle or feel; reach with hands and arms climb or balance and stoop kneel crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds. The employee must be able to be outdoors in various weather conditions for extended amounts of time. The ability to cope with and tolerate high levels of stress is also a necessity. Specific vision abilities required by this job include close vision distance vision depth perception and ability to adjust focus.

Ability to maintain compliance/certification with CCHOs requirements for using physical restraints (SCM: Safe Crisis Management) including sufficient strength and completion of extensive training. Physical ability to respond to crisis including but not limited to: running and performing physical restraints.
