Job Summary:

The Tele Discharge Coordinator is a systemwide role supporting coordination and delivery of high quality patientcentered care. The Tele Discharge Coordinator is responsible to prepare the patient both physically and mentally for the end of hospital care and return to previous lifestyle activities. The Tele Discharge Coordinator utilizes remote technology to support the efforts of the primary nurse at the bedside and other onsite staff. The discharge process involves the patient family caregivers and hospital and healthcare teams.

The Tele Discharge Coordinator supports the discharge process for patients across all Virtua hospitals. The work location will require working in a command center outside of a hospital; and/or may require working remotely.

Position Responsibilities:

Participates in discharge preparation of patient via virtual conference to provide education for selfcare and knowledge of home medications; communication to family/caregiver regarding discharge plan.

Provides education to patients/families/caregivers through active teaching by virtual conference. Explains patient medical condition patient status treatment plan medications and possible side effects and followup measures. Translates complex medical terminology to ensure complete understanding.

Recognizes and reports errors and unusual occurrences. Analyzes and identifies precipitating factors.

Provides mentorship and peer support to clinical nurses as needed.

Consults virtually with a multidisciplinary care team to coordinate evaluate customize and deliver a holistic patient discharge plan that promotes and supports recovery.

Uses clinical judgment in collaboration with a multidisciplinary care team to recommend continuum of care programs after discharge.

Maintains compliance in confidentiality adheres to HIPAA and PHI in work from home standards.

Serves as an example of professional nursing practice and promotes pride in nursing for the organization the community and the health care industry.

Participates as an engaged active member of the health care team and positively promotes the rest of the team to patients/families/caregivers and other colleagues.

Maintains competencies for using and troubleshooting as appropriate all required remote technologies.

Maintains a professional appearance and demonstrates behaviors consistent with established policies/procedures.

Position Qualifications Required:

Required Experience:

Minimum of 3 years of acute care nursing experience 5 years preferred. Excellent communication skills assessment skills and clinical judgement required.

Required Education:

Graduate of an accredited nursing program.

BSN preferred.

Training / Certification / Licensure:

Current State of New Jersey nursing license to practice as a registered nurse. Current CPR certification BLS. Relevant certification preferred.
