DUTIES/RESPONSIBILITIES
- Provides primary medical care with physician supervision for pediatric and adult patients
- Performs physical exams diagnosis including ordering and interpretation of diagnostic tests and treatment consistent with licensed scope of practice and granted privileges
- Consults with supervising physician when necessary to prepare comprehensive patient care plan
- Appropriately refers patients to physician for consultation or to specialized health resources for diagnosis and treatment as needed
- Ensures the delivery of all medical care is consistent with regulatory accreditation and professional standards including clinical policies and quality initiatives
- Participates in the review and development of clinical policies protocols and standing orders
- Actively engages in compliance and quality improvement activities including but not limited to peer review patientcentered medical home UDS reporting PDSA cycles SOAP notes other valuebased care projects
- Attends all scheduled provider and allstaff meetings and trainings unless excused by CMO
- Ensures accuracy timeliness and appropriate confidentiality with patient electronic documentation per contract
- Works evening and weekend hours as scheduled and shares afterhours call coverage with other providers
- Conducts all scheduled clinic shifts in person at assigned clinic location (Northview Heights and/or Middle Street offices) and conducts virtual visits as needed.
- Develops and ensure ongoing positive relationships and collaboration with all health center staff
- Meets professional behavioral expectations for attendance appearance professionalism communication
- Works within a multidisciplinary team
- Identifies barriers to selfmanagement plan and meeting goals
- Trains mentors and supervises students as assigned
- Consistently practices universal precautions
- Potential exposure to biohazardous materials
- Prolonged sitting and/or standing and moving about throughout the workday
- Willingness to participate in a minimum of 2 hours of community outreach per year
- Is punctual and ready to participate in clinic care at start of each clinic session
- Participates in morning patient care huddles up to 10 minutes outside of patient care hours
- Comfort working with the NSCHC faithbased mission to provide care for all who request it without judgement
KNOWLEDGE/SKILLS/ABILITIES
- Ability to establish and maintain positive caring relationships with managers physicians advanced practice providers ancillary and support staff other departments and patients/families
- Ability to work productively and effectively within a complex environment and with a multidisciplinary team
- Maintains requisite CME credits for area of specialty
- Strong computer skills including Word Excel and PowerPoint
- Requisite skills and ability to perform certain medical tasks as assigned
- Demonstrate a high level of problem solving skill to better serve patients and staff
- Ability to utilize computers for data entry and information retrieval
QUALIFICATIONS (REQUIRED FOR EMPLOYMENT)
- MSN or DNP from an accredited educational institution
- Licensed to practice in Pennsylvania
- Current Nurse Practitioner certification
- Able to care for pediatric and adult patients including newborn and geriatric patients.
- Active DEA license required
- Minimum 5 years of experience working as Family Medicine Nurse Practitioner with Board Certified Physician supervision
- Current provider card in Basic Life Support (BLS)
- Clearances: Act 33 34 73
- Annual COVID vaccination encouraged
- Annual influenza vaccine required
- Must obtain or provide proof of (within 14 days of hire): PPD Tdap Hepatitis B immunity MMR immunity varicella immunity
