Purpose
Under the purview of the Permanent Supportive Housing Department the Permanent Supportive Housing Case Manager provides case management services to adults experiencing homelessness who will be living in longterm supportive housing arrangements. The case manager may assist with finding securing and maintaining housing while also providing linkage to medical mental health and substance abuse resources entitlement/state benefits assistance and employment opportunities.

Salary range:$25.50 $27.50
