Job Description
Overview
Schedule: MondayFriday 9am5pm. Will also participate in an oncall rotation with some evening that will be required.
- Provide services to clients and their families including case planning education and advocacy
- Participate with staff team in making decisions regarding client care
- Facilitate referral interviews intakes case conferences family team meetings or other meetings as assigned
- Prepare and maintain complete case files and documentation for all clients
- Assist with and participate in ongoing and followup reviews to evaluate appropriateness and quality of care provided.
Job Skills / Requirements
- Bachelors degree in human services social work or a closely related field
- At least 2 years of experience working with youth and families
- Leadership experience is preferred
- Must have a passion for helping youth in crisis
- Ability to work collaboratively with other professionals in the organization and the community
- Must have excellent written and oral communication skills
- Must have excellent organizational skills and computer/data entry experience
- Valid drivers license and a good driving record
