The Jail Care Coordinator embedded within the Medina County Jail is a dedicated individual in the Mental Health Grant Program for JusticeInvolved Individuals (MHGJII). In this role you will provide crisis intervention services as needed along with providing planning and case management for inmates with mental health and/or substance use conditions who are eligible for pre and postrelease from the jail. This position would also act as a liaison between the County Jail HCMHDD Centers and local community resources to ensure continuity of care for individuals with the intention of reduction in recidivism rates related to mental health and substance use conditions. Duties include coordinating behavioral and physical health needs while in jail and upon release from jail; providing behavioral health consultation and follow up with primary care and behavioral health care patients. The delivery of services will be based on the needs of the community and individuals experiencing a mental health need. The Jail Care Coordinator will assess specific needs to be able to develop and implement Person Centered Recovery plans that support individuals with mental illness/substance use in learning skills to reach their specified service outcomes and to attain/maintain individual recovery. The Jail Care Coordinator will complete preintake needs and postrelease assessments. The Jail Care Coordinator will follow cases for up to 90 days postrelease to ensure connection to the appropriate level of care in the community. The Jail Care Coordinator must provide timely and accurate documentation that supports diagnosis and outcomebased services. This position collaborates with intra and interagency staff to improve services for the community. The Jail Care Coordinator exercises extensive independent judgment and will help to maintain data which will be used to plan and make projections for current and future services to meet outcomes. Duties of the position will require frequent interactions with jail staff community members Primary Care Physicians hospital and Hill Country MHDD program staff. Requires flexible hours involves extensive paperwork and will require selfdirected performance. Must have a valid Texas drivers license and a driving record that is acceptable for insurability through the Centers insurance carrier and pass a criminal background check. Will participate in community coalition meetings trainings and Criminal Justice training seminars as designated.Supervised by the MHDD Adult Mental Health Director.
