We are seeking a passionate and dedicated Care Team Coordinator to join your team. In this role you will coordinate and schedule home and community visits communicate with our healthcare providers ensuring seamless service delivery while providing empathetic support to the clients and providers. Your administrative skills will help keep our operations organized and efficient.
