JOB DEFINITION:
Work with the Central Nassau FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.
EDUCATIONAL REQUIREMENTS:
Licensed Clinical Social Workers Licensed Marriage and Family Therapists or Licensed Mental Health Counselors.
EXPERIENCE REQUIRED:
Three years experience in mental health and/or chemical dependency field.
DUTIES AND RESPONSIBILITIES:
1. Charged with supervisory and administrative responsibilities to ensure that staff meets the standards and requirements of the Agency and appropriate funding and regulatory bodies.
2. Direct and supervise staff activities lead team organizational and service planning meetings and provide direction to staff regarding individual cases. Conduct sidebyside contacts with staff and regularly conduct individual supervision meetings.
3. Responsible for overseeing Assessment and Service Planning Quality assurance and Improvement Processes Utilization Review Incident Reporting coordination between FACT and other systems.
4. Engage in performing assessments handling crisis intervention and discharge planning.
5. Provide direct facetoface and/or collateral services to recipients either in the FACT office or in the community in places and situations where problems arise.
6. Must be available (along with other members of the FACT Team) on an as needed basis 24 hours a day 7 days a week. Manages the afterhours calls for the team.
7. Participates in planning for the needs of the community and the Agency with the rest of the administrative team.

9. Identify and implement quality improvement trainings and activities as part of professional development goals

13. Other duties as assigned.
