DUTIES/RESPONSIBILITIESMay include the following: Transportation Duties: - In coordination with the Case Manager assess clients needs for transportation to medical social service appointments.
- Inform Case Manager of all transportation arrangements and changes in transportation arrangements in a timely manner.
- Coordinate multiple client transportation to reduce staff and travel time.
- Schedule client appointments and maintain a transportation calendar.
- Call clients to remind them of scheduled appointments.
- Cancel clients appointments as needed.
- Maintain good working relationships with medical and dental providers.
- Document client contact in transportation log within 72 hours of contact.
- Maintain documentation of travel.
- Adhere to all Community Action Transportation Procedures.
- Ensure that all clients understand agree to and sign a copy of the Safe and Confidential Transportation Practices.
- Deliver food bank services to clients as assigned by Case Manager.
- Deliver medication to clients as appropriate and adhere to Medication Transportation Procedures.
General Duties: - Submit original receipts and bills to your supervisor weekly.
- Maintain and submit accurate travel records and time vouchers.
- Maintain and care for agency vehicle: wash car twice a month if needed schedule and complete regular scheduled maintenance vacuum the vehicle once a month remove and trash or personal items from the vehicle after each transport and inform your supervisor of any concerns regarding the maintenance care and operating of the vehicle.
- Maintain vehicle first aid kit
- Must assure confidentiality of case records referrals and information concerning clients.
- Inform supervisor of any client violations of the Safe and Confidential Transportation Practices.
- Be active in ongoing self and program assessment in order to participate in continuous improvement of your own skills and program enhancement.
- Report all suspected abuse neglect and exploitation of children elderly and the disabled to the Abuse Hotline atalong with your supervisor and Health Services Director.
- Meet regularly with your supervisor to seek support and ensure the quality of the work.
- Participate in annual training on the following topics: Confidentiality Ethics and Cultural Competency
- Other duties as assigned.

REQUIREMENT/QUALIFICATIONSPreferred: - Previous experience transporting clients
Required: - At least 21 years old
- Valid Texas Drivers License Class C
- Vehicle Liability Insurance and ability to meet underwriting requirements for agency liability insurance
Physical Requirements: - Be able to lift 20 pounds
