Description

If youre dedicated to making a positive impact on the lives of community members with developmental disabilities explore this exciting opportunity. The Developmental Disabilities Administration is looking for a Transitional Case Managerto join our team in Seattle WA. In this role you will provide specialized transitional case management and resource assistance for people with developmental disabilities transitioning from one setting to a new one. You will work with families and community members to advocate on behalf of clients.

This position could be based out of the Kent DDA office as well as Seattle/Cherry Street office. A hybrid telework schedule is available for this role in our office location in Seattle/Kent WA.

Some of what you will do


We are looking for professionals who have
  • A bachelors degree in social sciences social services human services behavioral sciences or an allied field plus 2 years of experience providing social services to people with developmental disabilities
    Or
    Satisfactory completion of 12 months as a Case Resource Manager Trainee
  • Ability to identify support needs and plan for appropriate services for persons with developmental disabilities
  • Possess a high degree of effective communication skills (write listen observe communicate interview understand confront engage in problemsolving and resolve conflict)
  • Assess and identifysocial dynamics between clients and families and/or other relationships
  • Ability to identify and notify appropriate entities whenclients and/or coworkers may be at risk
* Graduate school training in the areas listed above will substitute for one year of experience providing social services to people with developmental disabilities.
