Job Description:
- Position:RN Care Manager
- Location: Outpatient Clinic
- Scheduler:FullTime
- Signing Bonus: $5000
Responsibilities:
- Maintain and improve the case management system using ongoing interaction with patients physicians and other health providers to meet designated clinical operational and financial outcomes for aggregate patient populations.
- Demonstrate a solid understanding of all regulations and standards associated with Case Management that CWHS is held to.
- Assist physicians patients families staff and payers in providing care for patient populations that is appropriate effective and cost efficient.
- Attend and actively participate in all departmental and interdepartmental meetings related to Case Management/Social Services/Discharge Planning functions.
- Assist in performing Medicare Annual Wellness visits.
Requirements:
Bachelors degree in nursing (preferred)with at least three(3 years of clinical experience and at least two 2 years of case management experience; certification in case management (preferred)
OR
equivalent combination of education and experience
Current unrestricted Registered Nurse license in the State of Colorado (required)
Willing to obtain Case management certification within 3 years.
Compensation:
- $38.00 $43.70 per hour depending on education and experience.
