Job Description

The Clinical Data Abstractor is responsible for medical record chart abstraction and data entry functions to meet state federal and Joint Commission regulatory requirements. Follows all guidelines for abstracting function. This individual also serves at the Stroke Program Coordinator is responsible for the ongoing maintenance evaluation and updating of the St. Lukes Warren Hospital Primary Stroke Center program. Through collaboration with the Stroke Program Medical Director and the SLUHN Stroke Program Coordinator this individual is responsible for of Stroke Center operations clinical practice guidelines performance improvement initiatives staff development and collaboration with the EMS and community outreach activities.

JOB DUTIES AND RESPONSIBILITIES:

  • Maintains a current knowledge of the abstracting guidelines guidelines for data entry and specification manual updates and revisions for core measures (IP & OP).

  • Maintains a current knowledge of computer software upgrades and applications.

  • Achieve 80 or greater validation audit scores for CMS and Joint Commission core measures.

  • Acts as technical support liaison between third party vendors (such as Nuance and Outcomes Science) and St. Lukes Warren Hospital.

  • Assists with the education of nursing and medical staff regarding quality measures.

  • Identifies and reports any trends that could affect patient outcomes and quality measure scores.

  • Works with nursing to develop new forms that assist with meeting the requirements of the quality measures.

  • Facilitates consistent standards of care of stroke patients through evidencebased clinical practice guidelines. Assists in the development and updates to policies and procedures care guidelines clinical pathways and educational materials as appropriate.

  • Improves the quality of care for the stroke patient population by facilitating process improvement plans to ensure compliance with clinical practice guidelines. Facilitates the St. Lukes Warren Campus Stroke Committee. Provides data for the analysis to determine needs for improvement in clinical practice. Communicates appropriate care information to multiple level of care providers.

  • Assists Patient Care Managers Clinical Coordinators and Clinical Educators in fostering the development of staff involved in the care of stroke patients to promote recovery and optional patient outcomes.

  • Participates in community education when needed. Facilitates programs to promote stroke awareness. Acts as a resource to St. Lukes Network affiliates and the community.

PHYSICAL AND SENSORY REQUIREMENTS:

Sitting for up to 6 hours per day 2 hours at a time. Walk up to 2 hours per day; 20 minutes at a time. Frequently uses fingers for typing data entry etc. Frequent use of hands. Extensively uses mouse as a computer entry device. Uses upper extremities to lift up to 10 pounds. Rarely stoops bends or reaches above shoulder level. Must be able to hear as it related to normal conversation. Must be able to see as it relates to general near far color and peripheral vision.

EDUCATION:

Registered Professional Nurse with current license to practice in the State of New Jersey or Pennsylvania. Bachelors Degree in Nursing or healthcare related field is preferred.

TRAINING AND EXPERIENCE:

Minimum of 5 years of fulltime clinical care nursing experience as a Registered Nurse in an acute care hospital setting. Experience preparing comprehensive written reports and making effective training/education presentations. Works well in a changing environment. Demonstrated success in evaluating quality and cost outcomes highly desirable. Excellent project management skills.
