Job Title: Social Services Assistant
Location: Garden City Healthcare Center Modesto CA
Facility Type: Skilled Nursing Facility
Schedule: FullTime Monday Friday
Compensation: Competitive Pay (DOE)

Garden City Healthcare Center is a compassionate and residentcentered skilled nursing facility located in Modesto CA. We are currently seeking a caring organized and driven Social Services Assistant to join our interdisciplinary team.
Position Summary:
The Social Services Assistant supports the Social Services Director in helping residents and their families adjust to life in the facility. The ideal candidate will demonstrate strong interpersonal skills attention to detail and a genuine passion for serving the elderly population.
Key Responsibilities:
Assist with psychosocial assessments and the development of individualized care plans.
Support residents and families during admissions transfers and discharges.
Coordinate services with community agencies and help arrange outside appointments and transportation when necessary.
Document progress notes and maintain accurate social services records in accordance with facility policies and state/federal regulations.
Participate in care plan meetings and collaborate with the interdisciplinary team.
Provide emotional support to residents and families including crisis intervention when needed.
Promote residents rights and advocate for quality care and services.
Qualifications:
High school diploma or equivalent required; Associates or Bachelors Degree in Social Work Psychology or related field preferred.
Experience in a longterm care or healthcare setting strongly preferred.
Excellent communication and documentation skills.
Strong organizational and time management abilities.
Compassionate patient and able to handle sensitive situations with professionalism.
