Job Title: Wellness Specialist WellLIfe

Position Reports to Job Title: ACT Team Leader

Location: Smithtown NY

Position Summary:

Demonstrate adequate knowledge of DSM V and psychiatric diagnoses to provide comprehensive assessment and treatment planning to individuals with severe mental illness. Wellness specialist provides weekly individual group and family therapy services in home and communitybased settings. Wellness Specialist also has lead responsibility for providing wellness services including wellness psychoeducation and community integration groups. Work environment requires flexibility with on and off site work locations and the ability to work individually as well as part of an interdisciplinary team.

Essential Accountabilities:

  • Control the quality of services through regular formal supervision case conferences and case consultation to ensure a high standard of services.
  • Develop treatment plans with other members of the ACT team and the consumers.
  • Complete comprehensive assessment and all addendums within 30 days on new admissions
  • Complete discharge summary attend discharge/transfer meetings and primarily responsible for all discharge followups
  • Review client chart every 6 months and upon discharge from psychiatric hospitalizations and complete paperwork as indicated
  • Obtain releases for family and other involved collaterals including documenting any refusals.
  • Primary responsibility for inputs into GSI for caseload including monthly contact note HML plans of care and other tasks TBD
  • Complete brief eligibility assessment for all HARP clients yearly
  • Primary responsibility for input into CAIRS upon admission annually (biannually for AOT) and discharge.
  • Attend 4 team meetings per week
  • Coordinate with family housing AOT probation and other nonmedical collateral contacts
  • Coordinate with hospital for psychiatric hospitalizations including attending discharge planning meetings
  • Complete AOT weekly reports AOT pick up orders significant event and incident reports.
  • In collaboration with other team members provide clients with monthly schedule as indicated
  • Identify clients ready to transition and provide referrals
  • Assist in planning organizing and attending monthly socialization/wellness group on a rotating basis
  • Participate in 24/7 oncall rotation
  • Ensure all documentations are completed in a timely fashion and remain in compliance with agency & OMH regulations.
  • Participate in OMH approved ACT Orientations and Trainings.
  • Ability to operate an agency vehicle.
  • Attend any scheduled trainings relevant to the position or deemed necessary by Team Leader.
  • Participate in activities of other staff members in their absence or during periods of staff shortage.
  • Maintain confidentiality at all times.
  • This does not limit the assignment of duties or exclude the performance of other duties not mentioned as assigned by supervisor
  • Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.
  • Participate in on call rotation to respond to emergencies 24/7.

Qualifications:

Masters Degree in social work or related field. Experience providing services to adults with severe mental illness preferred. Computer proficiency including the ability to use email and electronic medical record for all documentation. A valid NY State Drivers License required.
