Sentara Health is seeking a dedicated Integrated Nurse Case Manager/RN/OBGYN Western Tidewater region of Virginia!
This fulltime workfromhome position requires travel to conduct facetoface home visits in the members home within the Western Tidewater region of VA.
This includes but is not limited to the following areas: Franklin Suffolk Isle of Wight County (Smithfield Windsor) and Southampton County (Courtland Capron Ivor).
Applicants must reside in or near one of these locations to be considered for the role.
Education
- BSN (Bachelor of Science in Nursing) preferred
Certification/Licensure
- Registered Nurse License (RN) Nursing License Compact/MultiState License require
Experience
- Nursing experience with a focus on case management care coordination or patient advocacy Medical/Clinical cases for pregnant and postpartum women.
- Maternity experience (L&D Postpartum Mother/Baby) is experience (L&D Postpartum Mother/Baby) is preferred.
- Strong knowledge of physical psychological sociocultural and cognitive patient needs.
- Excellent communication skills both oral and written as well as strong problemsolving and analytical skills.
- Ability to work independently and collaboratively in a remote fastpaced environment.
