Scheduled Weekly Hours:
32Gundersen is now hiring a part time Social Worker for ambulatory care!
What you will work:
.8 FTE 64 Hours every 2 weeks
Business hours MF between 85pm no weekends or holidays
Location: This position is located in the La Crosse main clinic
Location: Emergency Services La Crosse
What you will do:
Ambulatory social workers provide a variety of services to our patients in an outpatient setting including preoperative discharge planning care coordination care navigation resource connections supportive listening ACP completion and longterm care planning
Although based in La Crosse this position assists with service needs in all our WI clinics and other areas as needs arise

What you will need:
Bachelorsdegree in Social Work or in a related field
WI Social Worker license
It is desired candidates have 1 year work experience in social services in a hospital medical clinic county social service agency or nursing home.
