Jefferson Healthcare is an innovative and awardwinning healthcare system and we are hiring an experienced fulltime Ambulatory Clinical Pharmacist to join our interdisciplinary team and help expand our primary care services.
As a dedicated Ambulatory Pharmacist at Jefferson Healthcare your skills and knowledge will be essential in collaborating with our primary care team and meeting the needs of our patients while making a meaningful impact in our community. Providing medication and chronic disease state management while optimizing medication therapy and safety for patients. Become an integral member of the primary care team while practicing under a progressive collaborative drug therapy agreement.

Schedule: 1.0 FTE; 40 hr/week; Day Shift; Monday Friday

What we are looking for:
- 5 years of ambulatory care pharmacy experience preferred.
- PharmD from an accredited school of pharmacy required.
- PGY1 residency or two years experience in the ambulatory clinical pharmacy field required.
- PGY2 ambulatory care residency preferred.
- Ambulatory Care Board Certification preferred.
- Current Washington State Pharmacy License in good standing.
