Position Summary:
The HospitalBased Social Worker will provide direct services to individuals affected by violence. The HospitalBased Social Worker will be based at one of the designated hospital locations.
The social worker will assess client needs develop and implement treatment plans and provide ongoing evaluation of services. Reporting directly to the SVP CommunityBased Services the successful candidate will have a strong commitment to public service.
Duties & Responsibilities
Provide direct clinical services to clients including assessments counseling and crisis intervention.
Develop implement and evaluate individual treatment plans based on client needs.
Participate in multidisciplinary team meetings and case discussions.
Enter direct and indirect services into Simple Practice and documentation into Tower Health EPIC system as needed within 23 business days from the service.
Maintain additional thorough and timely grant documentation according to TLC and Tower Health regulatory standards.
Participate in community outreach and violence prevention and intervention initiatives.
Collect client information to begin case files and case plans as well as conduct intake assessments.
Coordinate services for clients including counseling medical care legal assistance and other necessary resources.
Advocate for clients and act as a liaison with community providers and agencies.
Foster and promote communication between Tower Health TLC community resources.
Provide crisis intervention and deescalation as needed.
Identify clients and family members in need of additional trauma support and to provide prevention referral and treatment services.
Provide homebased services to the families of clients as needed.
Adhere to ethical and legal standards of social work practice.
Participant in ongoing professional development and training.
Perform other duties as assigned by the SVP CommunityBased Services.
Skills & Knowledge:
Experience providing clinical services in violence intervention and traumainformed care
Experience in case management and coordination of client care
Strong understanding of the psychosocial aspects of violence and trauma
Excellent oral and written communication skills
Ability to work with diverse groups and in challenging situations
Strong organizational and case management skills
Ability to work effectively on a multidisciplinary treatment team
Knowledge of community resources and networking ability
Proficiency in Microsoft Word Excel PowerPoint and Outlook
Understanding of confidentiality and ethical guidelines in clinical services
Education & Experience:
Masters degree in social work.
Current license: either Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
Minimum of 2 years of experience in violence intervention or traumainformed care.

Hours of Work:
This is a fulltime 40 hours per week position. Hours to include 11am7pm Monday through Friday.
Work Environment:
Hospitalbased out of designated Tower Health hospital location with regular travel in the community and other locations.
Travel:
Travel to hospital sites patient homes community settings and other locations.
