SPECIAL EDUCATION CASE MANAGER (GRADES K12 Long Term Care and Treatment (LTCT)
Classification: Licensed 190 day calendar
Reports to: Long Term Care & Treatment Director
FSLA Status: Exempt Employee

POSITION OVERVIEW

This critical position consults and creates with the principal and LBLs Special Education Director.
Our students in LTCT are some of the neediest we serve and this position ensures access to education while they focus on their greatest concern in their young lives: their mental health. Supporting teachers as they work with students dealing with social emotional behavioral issues and mental health needs is one of the most fulfilling teaching experiences possible. This position provides the expertise resources and infrastructure to comply with all state and federal expectations
of IDEA. This position provides experiences that are unique and challenging while offering ongoing learning and
growth.

QUALIFICATIONS

1. Valid Oregon Teacher Standards and Practices Commission (TSPC) teaching license is required with Special Education Generalist endorsement: K12
2. Effective oral and written communication skills.
3. Knowledge and understanding of the mental health needs of youth.
4. Such alternatives to certain above qualifications may apply as deemed appropriate by the Superintendent and/or Board of Directors.

GENERAL INFORMATION

This position has the following expectation and prerequisite abilities.

2. Maintain a high level of confidentiality in accordance with HIPAA FERPA and adherence to the competent and ethical educator practices.
3. Maintain compliance with state and federal laws governing all aspects of our agency including ORS OAR IDEA FOIA and CIPA.
4. Exceptional prioritization of tasks timely completion of work assignments ability to take the initiative and work independently with minimal supervision.
5. Demonstrates respectful flexible and effective teambuilding skills of cooperation communication and active participation.
6. Present self and LBL in a professional manner including: in oral and written communication professional disposition when working with others and maintaining professional dress standards.
7. Ability to understand and follow oral and written directives find requested data and present in userfriendly formats for internal and public distribution.
8. Maintain regular attendance at work. Be punctual in meeting deadlines attending meetings and following schedules.
9. Ability to lift carry at least 25 pounds.
10. Ability to sit for extended periods of time in a vehicle or work setting.
11. Ability to safely maneuver self and others in an unpredictable environment.
12. Utilize teleservice platform to provide services to educators students and district staff as directed.

ESSENTIAL FUNCTIONS

This is not necessarily an exhaustive or allinclusive list of responsibilities skills and duties associated with this position. The supervisor of this position may modify this list at any time its deemed necessary.

1. Assess student knowledge skills performance behavior and attendance and track appropriately.
2. Create and maintain procedures that comply with all IDEA and ODE procedural requirements.
3. Determine education needs of individual students and provide relevant instruction.
4. Prepare and implement Individualized Education Programs (IEPs) to meet the defined needs of identified students including evaluating and report student progress.
5. Cooperate with the Trillium campus model regarding service delivery per the expectations of a secured controlled and locked environment.
6. Implement behavior supports and plans based on the students established treatment program and in cooperation with Trillium staff.
7. Record and report data according to established policy and state standards.
8. Use appropriate educational principles while working with students in the area of students interpersonal relationships.
9. Prepare specially designed instruction lesson plans with individualized learning activities and strategies for students including modifying and providing accommodations for instruction.
10. Participate in Individualized Education Program (IEP) and team meetings as assigned.
11. Complete all special education reporting to comply with ODE and federal mandates including December Census June Exit and Child Find.
12. Complete all special education intakes and file reviews.
13. Complete all special education records requests.
14. Collaborate with the transition specialist to support students transition back to resident school district or next placement..
15. Effectively use word processing databases and online programs.
16. Provide referral of students to other agencies and specialists as appropriate.
