Community Liaison- Home Health

As we continue to grow we are seeking a dynamic and driven Community Liaison to serve as a key connection between our agency and the communities we serve.

Position Overview:

The Community Liaison is responsible for developing and maintaining referral relationships with physicians hospitals discharge planners assisted living communities senior centers and other community resources. This role plays a vital part in growing our patient base by promoting our services and ensuring excellent communication between referral sources and our clinical team.

Key Responsibilities:

• Cultivate and maintain strong relationships with healthcare professionals and community partners
• Educate referral sources and the public about our home health care services
• Identify new referral opportunities and generate patient referrals
• Represent the agency at community events health fairs and professional meetings
• Establish and maintain positive working relationships with new current and potential referral sources
• Collaborate with the office clinical team to ensure a seamless patient onboarding experience
• Track outreach efforts referral trends and maintain accurate documentation inEMR system as needed
• Provide excellent customer service and follow up with referral sources to ensure satisfaction
• Generate new referrals for home health by building new relationships with physicians longterm care hospitals discharge planners independent and assisted living facilities and other community resources.
• Maintain comprehensive working knowledge in the field of home health marketing

Qualifications:

• Our ideal candidate would have worked in a sales capacity 2 years for Home Health and/or Hospice Home Care Medical Equipment Company Pharmaceuticals etc. with established relationships or leads in the community
• Excellent communication presentation and interpersonal skills
• Ability to work independently and manage time effectively
• Must be willing to drive with reliable transportation valid drivers license and auto insurance
• Ability to maintain open communication channels with physicians hospitals and community partners as we seek to provide continued delivery of care throughout the service area
• Candidates will need to feel confident in their ability to be a selfstarter as well as work with an interdisciplinary team of highly skilled home health members
• Excellent research skills to identify trends and opportunities within the community
• Ability to market aggressively while simultaneously maintaining positive industry relationships
• Demonstrate of good communication negotiation and public relations skills
