Job Description

For more than 50 years First Choice Community Healthcare has provided access to high quality primary medical dental and behavioral healthcare to the underserved populations of the MidRio Grande Valley of central New Mexico. Today we operate seven 7 Community Health Centers and one 1 schoolbased clinic. We are currently recruiting for a Medical Records Clerk to join our team and assist in fulfilling our organizations mission which is to improve the health life skills and wellbeing of all members of the communities we serve. Our health centers maintain an opendoor policy providing treatment regardless of an individuals income or insurance coverage. As a nonprofit organization First Choice is a vital community resource in Central New Mexico. Not only does our organization provide the highest quality comprehensive medical services but we also address our patients emotional social and cultural needs.


This is a Temporary position without no benefits.



A. Position Summary

Under the supervision of the Health Center Manager assembles and maintains complete medical records according to established procedures. Files and retrieves patient records; prepares new files; may open and distribute mail.


B. Essential Duties And Responsibilities

  1. Sorts and files loose paperwork in patient charts; maintains medical records in proper order.
  2. Creates patient folders for new patients using unique identification numbers according to established protocols.
  3. Responsible combining nonoriginal charts with original charts;
  4. Retrieves patient charts on a daily basis for next day appointments and refiles charts in proper sequence;
  5. Completes out guides for pulled charts.
  6. Locates records that have been checked out or are missing in accordance with policies for safeguarding patient records.
  7. Follows established departmental policies procedures and objectives continuous quality improvement objectives and safety environmental and/or infection control standards.
  8. May perform other routine clerical tasks. Is responsible for filing of xray EKG and other reports according to health record order of assembly format;
  9. Checks for errors in patients identification;
  10. Pulls and screens health records for completeness makes patient identification cards and refiles records upon completion of activities;
  11. Responsible for sending medical records to appropriate destination either through fax phone or mail;
  12. Responsible for charging appropriate fees if any for copies of medical record(s) and submit to Central Billing for further processing;
  13. Where dental services offered completes dental charts for medical records;
  14. Checks encounter forms against medical record to assure conformity audits medical records to ensure adequate documentation to patient encounter;
  15. Checks encounter forms against signin list to see that no encounter forms are issued;
  16. Assures that all statistical reporting forms (family and individual registration forms and encounter forms) are checked for accuracy to see that all categories are filled in;
  17. Maintains daily census from signin list;
  18. Responsible for archiving medical records at the end of the year by labeling boxing and utilizing the computer system to place notes and further research in the future;
  19. Responsible for purging of health records;
  20. Obtains and releases medical information as directed by physician;
  21. Performs miscellaneous jobrelated duties as assigned.
Requirements

C. MINIMUM EDUCATION AND EXPERIENCE

  1. High School graduate or GED certificate;
  2. Three months related experience;
  3. Bilingual English/Spanish preferred

D. LICENSES/CERTIFICATIONS REQUIRED

  1. CPR Certification within 6 months of hire.

E. KNOWLEDGE SKILLS AND ABILITIES REQUIRED

  1. Ability to work as a team member within a clinical environment.
  2. Records maintenance skills.
  3. Ability to develop and maintain filing systems.
  4. Ability to complete routine paperwork.
  5. Ability to maintain confidentiality of records and information.
  6. Skill in the use of operating basic office equipment.
  7. Ability to follow routine verbal or written instructions.
  8. Ability to maintain quality safety and/or infection control standards.
  9. Ability to maintain patient confidentiality.

G. COVID19 VACCINE REQUIRED

H. PHYSICAL CHARACTERISTICS/WORKING CONDITIONS

A person in this position has sufficient time to complete most tasks although under pressure depending on patient and organization demands. There are frequent opportunities to relax from any physical exertion or to change position in work activities.

  1. Physical Effort and Dexterity: Good dexterity to operate personal computer and office equipment. Occasional lifting and carrying related to office duties.
  2. Machines Tools Equipment required to be operated: Capable of using office machines and personal computers for word processing data entry and spreadsheet applications.
  3. Visual Acuity Hearing and Speaking: Must be able to read a computer monitor and outputs accurately. Must be able to communicate clearly and accurately for work and safety compliance.
  4. Environment/Working Conditions: Work is mostly inside an office in a controlled environment. Normal office safety precautions and practices are required. Position requires travel throughout the Albuquerque metropolitan area. Work regularly scheduled MondayFriday although weekends may be required to meet deadlines
