Job Title: Rehabilitation Team Coordinator
Job Type:Full Time
Position Summary:
Works under the direct supervision of the Rehabilitation Therapy Services Team Leader and/or the leadership of the Inpatient or Outpatient Manager
Uses objective evaluation techniques to assess the physical limitations and medical condition of patients
Prescribes and implements a treatment program that is designed to return patients to their maximum level of function
Develops the patient care service to meet the standards of care Works collaboratively with all members of the multidisciplinary health care team to maximize patient and team outcomes
Promotes improvement in service department and hospital systems to benefit patient care Incorporates and operationalizes the goals and objectives of the Hospital at the site of care
Mentors others to promote quality and excellence of care Coordinates patient care activities on the service

Salary Range: Minimum$41.70/hour Maximum$68.85/ will be commensurate with equity and experience for roles of similar scope and responsibility.
