Job Description

Job Summary:
The Clinical Pharmacy Specialist for Advanced Heart Failure is responsible for the provision of pharmaceutical care by working with the advanced heart failure team in the care of patients with heart failure ventricular assist devices and heart transplants serving as the drug expert helping to manage the complex pharmacotherapy regimen of each patient. The clinical specialist actively manages and adjusts medication therapies via drug therapy management agreements with prescribers and helps manage drug costs by ensuring formulary and stewardship guidelines are followed. Responsibilities for the position include: comprehensive medication management interdisciplinary decision making emergency code response care coordination education policy/procedure development and research activities. They will provide drug information and education to patients students residents pharmacists and other health care practitioners.

Minimum Qualifications:
- Graduation from an accredited College of Pharmacy resulting in Pharm D. or BS degree.
- Completion of an ASHP accredited PGY1 residency program and PGY2 (preferably in Cardiology) residency program or equivalent experience is required.
- Current licensure as a pharmacist in the State of Maryland required.
- Board Certification through the Board of Pharmacy Specialties or a certification deemed appropriate for the practitioners practice setting by Division Director is required within 2 years of start date.
- Hospital and departmental education requirements completed within 90 days of hire. ACLS and/or PALS certification is desired.

Salary Range: Minimum$58.41/hour Maximum$96.46/ will be commensurate with equity and experience for roles of similar scope and responsibility.
