Position Overview: Physical Therapist (PT)
As a Physical Therapist at Pinnacle Home Care youll play a vital role in delivering highquality care to patients in the comfort of their homes. Youll work under the direction of a physician and nursing supervisor ensuring care meets all regulatory and clinical practice standards.
Key Responsibilities
- Perform initial and ongoing patient assessments including OASIS documentation.
- Develop and implement tailored treatment plans in consultation with physicians adjusting as needed.
- Collaborate with the interdisciplinary care team to optimize patient outcomes.
- Provide education and support to patients and families on therapy programs.
- Supervise Physical Therapist Assistants every 30 days.
- Communicate patient progress and changes with physicians and care teams.
- Document all visits and progress notes on the same day of service.
Qualifications
- Graduation from an accredited Physical Therapy program
- OASIS report experience preferred
- Active PT license in Florida.
- Current BLS certification.
- Valid drivers license and reliable insured vehicle.
- At least one year of PT experience (home health experience preferred).
- Proficiency with electronic medical records is a plus.
- Physical capability to perform required tasks (standing bending lifting).
- Strong selfmanagement communication and judgment skills.
