The mission of the Nurse Practitioners role at Dialogue is to accompany the patients in their journey with Dialogue by offering frontline medical support and symptom evaluation. Collaboration and learning are both significant aspects of this role. Dialogues multidisciplinary approach fosters teamwork continuous education and insight from a variety of medical professionals.
What youll be doing:
- Conducting remote assessments via the Dialogue Care Platform where you will evaluate symptoms diagnose and provide treatment for both common and complex conditions
- Delivering highquality patient care by offering timely medical advice and identifying urgent symptoms
- Creating personalized care plans tailored to your patients needs
- Collaborating closely with an interdisciplinary team of doctors nurses care coordinators and care managers psychologists etc to provide comprehensive highquality care
- Educating and supporting patients on preventative care health management and lifestyle changes
- Referring patients when needed to specialists or allied healthcare professionals and community services
- Maintaining detailed records of patient interactions treatment plans and followups that adhere to regulatory standards
- Contributing to internal projects or initiatives
Wed love to hear from you if you have:
- Must be comfortable consulting in French or English
- The availability to work evenings and weekends for a minimum of 8 hours per week
- Masters degree in Nursing advanced practice option and a complementary diploma in primary care
- Hold a valid and active medical practice license in Quebec Manitoba or Saskatchewan
- Desire to work in an environment that encourages autonomy professionalism and versatility
- Motivated by a work environment focused on continuous learning and excellence
