The University of Miami Health System UHealth Department of Pediatrics Hematology has an exciting opportunity for a FullTime Case Worker 2 to work in Miami.
The Case Worker 2 monitors outcomes data collection and record keeping and coordinates services for the respective population including providing information about professional development opportunities quality improvement strategies and counseling to improve the clients social and psychological conditions. The Case Worker 2 assists those experiencing difficult situations by helping them find the community resources/services that they need contributes to the development of family support plans and monitors its implementation and modification.
Core Responsibilities:
Conducts situational analysis which may be at home and/or office visits mentors and provides technical assistance as needed.
Collaborates with the service agencies and program leadership to provide and administer services appropriately.
Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
Maintains and reviews records and files to ensure compliance with project guidelines.
Collaborates with all funding sources to ensure the availability of adequate resources.
Maintains current knowledge of services available and changes applicable rules and regulations.
Attends meetings and training which may include the Early Learning Coalition United Way and the Quality Counts Career Center to enhance developmental potential.
Adheres to University and unitlevel policies and procedures and safeguards University assets.
Department Specific Functions:
Work as a case manager or care coordinator within the Division of Pediatric Hematology Oncology and specifically with children and young adults with diagnosis of sickle cell disease.
The employee will assess and act upon care coordination of referred cases by coordinating appointments getting approval of imaging and medications calling families and other necessary steps to coordinate or facilitate care.
The employee can proactively assess without having the referral from other providers what are the needs patients face to receive care.
The employee will have knowledge of health insurances and will assist patients and families in navigating through their health insurance assisting them to achieve the maximum benefit possible.
The employee will be part of a multidisciplinary team and will meaningfully contribute to it.

CORE QUALIFICATIONS:
Bachelors degree nursing communication social and behavioral sciences or other pertinent area
Minimum 3 years of relevant experience required
Certification and/or licensing in care coordination preferred.
Bilingual Spanish and Haitian Creole is a plus
Learning Agility: Ability to learn new procedures technologies and protocols and adapt to changing priorities and work demands.
Teamwork: Ability to work collaboratively with others and contribute to a team environment.
Technical Proficiency: Skilled in using office software technology and relevant computer applications.
Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.
Any appropriate combination of relevant education experience and/or certifications may be considered.
