POSITION PROFILE
The Nurse Specialized in Wound Ostomy and Continence (NSWOC) will provide and promote high quality client centered care while assessing and treating clients with complex wounds ostomies or continence conditions. The NSWOC uses evidence informed nursing knowledge skill and judgement and works within their scope of practice to provide and promote holistic nursing care to meet the physical safety emotional cultural and spiritual needs of clients and their families in their homes while mentoring and guiding the practice of other members of the Access Healthcare Services (AHS) nursing team. The NSWOC will refer and schedule assessment following the Ontario Health at Home (OHatHome) Guideline for NSWOC Assessments.
KEY RESPONSIBILITIES
- Completes inhome and virtual client assessments and develops and maintains collaborative client centred care plans accordingly
- Provides complete competent and relevant care to clients
- Adheres to best practice guidelines and Professional Practice Standards as outlined by the College of Nurses of Ontario (CNO) and Nurse Specialized in Wound Ostomy and Continence (NWOCC)
- Makes recommendations for devices tools or strategies that might improve the patients quality of life or outcome
- Promotes selfcare and autonomy of nursing clients
- Provides supervision mentorship and guidance to AHS nurses as required
- Completes the AHS NSWOC Assessment form for each consult
- Completes required referrals to interdisciplinary team as required
- Maintains and updates client care plans in Alayacare reflective of assessments and as client needs or goals evolve including strategies or interventions required
- Completes progress notes to flag care plan changes and recommendations to the AHSI nursing team
- Reports consults and updates members of the interdisciplinary health care team and OHatHome with clients status and treatment plan in a timely and professionalmanner
- Completes Automated Provider Reports to ensure funding from OHatHome is in place for visits and pertinent information is communicated with our funder.
- Acts as a mentor for the Wound Care Lead Team and supports in knowledge enhancement and development of team members.
- Works closely with the Clinical Manager to assess learning needs and identify educational opportunities for the nursing team.
- Supports the onboarding of nurses and provides ongoing education as needed or requested by the Director.
- Collaborates with the senior management team to review develop and implement new and existing wound care processes and policies.
REQUIREMENTS
- Registered Nurse specializing in Wound Ostomy Continence (NSWOC)
- Completion of a recognized Wound Ostomy Continence Nurse Education Program
- Current certification as Registered Nurse in good standing with CNO
- Proof of Covid19 vaccinations
- Must have a valid drivers license and access to a vehicle
- Clear vulnerable sector check
- Excellent assessment documentation verbal and communication skill
- Possess expert nursing skills in the provision of care to patients/clients/residents who have skin wound ostomy and/or continence challenges.
The skills and qualities we look for:
- 1 year community nursing experience preferred but not essential
- New graduates are welcome to apply
- Bilingualism considered an asset
