Scheduled Weekly Hours:
40Gundersen Health System (becoming Emplify Health) is looking for a fulltime Social Worker Complex Discharge Planner! This highly skilled Social Worker would have additional time to allow for increased collaboration with postacute care services county and/or legal resources physicians nursing and other interdisciplinary team members within the organization.
What you will work:
1.0 FTE 80 hours every 2 weeks
Monday Friday Days
One holiday per year
What you will do:
Ensure cooperation and coordination on a regular basis between all key stakeholders for their patients to allow for clear concise and timely discharge planning to occur.
Assessment and treatment planning
Discharge planning
Provide patient and family education and support
Documentation and communication Document all assessments interventions and services in the electronic health record.

What you will need:
Bachelorsdegree in Social Work or in a related human service field
WI Social Worker license
34 years of experience in a medical setting
