Work Location:Room 700 Rutland Building County Hall Leicester Road Glenfield LE3 Leicester Royal Infirmary.

Worker Category:Hybrid Worker & Field Based Worker &Shift Working Ability to work flexible hours on a rota basis based on the needs of the service between 8.00a.m 8. 00p.m. on 365 days a year rota including weekends evenings and Bank Holidays.

About the Role

10 x FTE Posts 9 x 37hour posts 2 x 18.5hour posts.

The councils HART service provides a number of support offers under its registration. This includes reablement which helps people return home from hospital or prevents them from being admitted to hospital and regain their independence.

The HART Reablement Support Workers work within Home First to identify and maximise reablement opportunities for people with a new or increased need because of an acute or community hospital admission.

The purpose of the job is to:

  • Support the triage of new discharge hub referrals identifying those who can return home following their hospital admission.
  • Complete a persons assessment on the ward/in their home to identify the goals for them to achieve or the support that is needed to remain safe at home and highlight any risk assessments to facilitate safe delivery of the service.
  • Work closely with health partners to promote a persons independence identifying any equipment or other working practices to ensure a safe and timely discharge.
  • Identify and support the immediate protection of potential safeguarding mental capacity continuing health care and other highrisk situations where an adults safety or liberty may be at risk informing supervisors as necessary so as to enable adults to take risks whilst protecting them from abuse managing the risks to staff and the council.
  • Provide a 365day service to maintain discharge flow/prevent hospital admission and achieve the most independent outcome for the person.

The Key Duties:

Working under the supervision and direction of the Team Leader/Team Manager to ensure high quality response to all adults who may require and/or be eligible for social care services by;

  • providing appropriate responses or referrals within required timescales organisational policy and legislative requirements
  • ensuring that responses are geared to ethnic religious cultural age and linguistic background practicing in a manner and delivering services which are antidiscriminatory
  • that concerns and complaints are reported to the TL/SCSW andare addressed by making appropriate changes to existing practices
  • providing prompt support to meet immediate risks and needs in crisis situations

To support the development of systems processes and procedures under the direction of the HART Manager to ensure services maintain comprehensive endtoend processes

Contribute to team effectiveness and response by working to Departmental standards and where joint working is in place those of the NHS.

Work closely with multidisciplinary team colleagues to provide a holistic joined up service which places the service user at the centre of this. This will be through a range of approaches including joint visits with colleagues and participation in regular MDTs. To work with health colleagues to deliver an integrated service where this has been identified as beneficial to the person. This includes working in acute and community hospital settings to support the triage of people who are medically optimised to leave hospital.

About You:

  • Level 3 Diploma in Health and Social Care or other equivalent qualification in a relevant subject or equivalent level of relevant experience. Relevant experience (paid employment training or voluntary sector) working with adults in a social care setting.
  • Experience of working with other agencies and/or working in a multiagency environment to identify and meet need with creative and achieve positive outcomes for service users.
  • A good understanding of the Councils Adult Social Care strategies relevant regulations/ legislation and service frameworks.
  • Ability to communicate clearly and accurately both orally and in writing with a wide range of people including representatives from other agencies using skills of negotiation and persuasion
  • The ability to use Information Technology applications e.g. Microsoft Outlook Word Excel and database applications
  • Able to deliver a sustainable high service through understanding and keep to departmental and service ability to prioritise effectively across a wide range of competing demands to meet deadlines
  • Ability and willingness to travel and work from various locations throughout the County and travel between sites as required.

A flexible approach to work access to a vehicle and drivers licence and an Enhanced DBS is required.
