Job Description

We are seeking a dedicated Behavioral Health Case Manager II to join the our states client department. The incumbent will work onsite with Delaware State Health Human Services teams to provide case management and facilitate treatment enrollment for individuals referred. This role is essential in supporting individuals dealing with substance use and mental health challenges.

Key Responsibilities:

  • Act as the primary behavioral health clinician for the Police Diversion Program.
  • Coordinate case management services and ensure seamless treatment enrollment.
  • Collaborate closely with Delaware State Police personnel.
  • Provide timely crisis intervention and effective case management.
  • Maintain accurate documentation of all client interactions and program activities.

Minimum Qualifications:

  • Bachelors or Masters degree in behavioral sciences counseling or a related field.
  • Demonstrated experience in substance use and mental health counseling.
  • Strong background in case management.
  • Ability to effectively collaborate with law enforcement agencies.
  • Teamoriented with excellent communication skills.
  • Proven ability to manage crises effectively.
