Job Description
Overview
Performs specialty functions in support of medication reconciliation. Obtains a current medication list for admitted patients through inperson interviews. Performs second sourcing by collecting medication list and verifying this with a second source.
Qualifications
Education:
- High School Graduate or equivalent required.
Experience:
Minimum of six months experience in healthcare preferred.
Other Knowledge/Skills/Abilities:
- Demonstrates excellent verbal communication skills.
- Demonstrates excellent interpersonal skills.
- Ability to input data accurately and efficiently into the Cerner Health Information System.
- Effectively utilize drug information resources.
- Maintain confidentiality of patient information.
- Ability to set priorities.
Responsibilities
- Conducts scripted interviews with patients admitted to the hospital.
- Obtains the patients most accurate and current home medication list including prescription and nonprescription medications.
- Obtains the patients medication allergy history.
- Resolves home medication list discrepancies with an inpatient hospital pharmacist and/or an MMC approved second source of information.
- Accurately documents the home medication list and allergy information in the Cerner Health Information System.
- Performs all other duties as assigned.
