Job Description

Overview

Performs specialty functions in support of medication reconciliation. Obtains a current medication list for admitted patients through inperson interviews. Performs second sourcing by collecting medication list and verifying this with a second source.

Qualifications

Education:

  • High School Graduate or equivalent required.

Experience:

Minimum of six months experience in healthcare preferred.

Other Knowledge/Skills/Abilities:

  • Demonstrates excellent verbal communication skills.
  • Demonstrates excellent interpersonal skills.
  • Ability to input data accurately and efficiently into the Cerner Health Information System.
  • Effectively utilize drug information resources.
  • Maintain confidentiality of patient information.
  • Ability to set priorities.

Responsibilities

  1. Conducts scripted interviews with patients admitted to the hospital.

  1. Obtains the patients most accurate and current home medication list including prescription and nonprescription medications.

  1. Obtains the patients medication allergy history.

  1. Resolves home medication list discrepancies with an inpatient hospital pharmacist and/or an MMC approved second source of information.

  1. Accurately documents the home medication list and allergy information in the Cerner Health Information System.

  1. Performs all other duties as assigned.
