Job Description
Job Title: Physician Family Practice/Internal Medicine
Specialization: Diabetes Care & Lifestyle/Wellness Specialist
Location: San Antonio Texas
We are seeking a compassionate and dedicated Family Practice or Internal Medicine Physician to join our innovative and patientcentered team. This role offers the unique opportunity to practice within a valuebased care model with a primary focus on diabetes care and the promotion of lifestyle medicine. Caring for a unique and rewarding patient population of First Responders you will have a direct impact on improving patient health outcomes by addressing not only clinical management but also the prevention of chronic disease through personalized lifestyle interventions. This is a fulltime 36hour a week position Monday Friday which requires traveling between 4 clinic locations (set schedule). No call and no afterhours charting. Provide comprehensive patientcentered care to individuals with diabetes managing both the clinical aspects of the condition and its associated comorbidities.
Develop individualized care plans that emphasize sustainable lifestyle changes including nutrition physical activity stress management and behavioral health.
Collaborate closely with a multidisciplinary team including dietitians diabetes educators clinical pharmacists and care coordinators to drive optimal outcomes and ensure holistic patient care.
Deliver evidencebased patientcentric care that integrates advanced diabetes treatments and medication management within a valuebased framework.
Empower patients and their families with knowledge about diabetes selfmanagement prevention and the transformative impact of healthy lifestyle habits on longterm wellness.
Maintain thorough uptodate documentation in accordance with organizational and clinical standards emphasizing patient outcomes and care quality.
Continuously engage in professional development and stay current with advances in lifestyle medicine and diabetes management to enhance care delivery.
Qualifications:
MD or DO degree BC/BE in Family or Internal Medicine.
Licensed or eligible for licensure in Texas
Expertise in diabetes care and a passion for integrating lifestyle medicine to improve patient outcomes.
Strong communication and interpersonal skills with a focus on building trusting longterm relationships with patients.
Experience working within a valuebased care model and proficiency in electronic health records (EHR) systems is preferred.
