Case Worker Concurrent Outpatient Medical & Psychosocial Addiction Support Service (COMPASS)


COMPASS is currently recruiting a fulltime temporary 6 months) Case Worker. COMPASS is an integrated research/clinical service incorporating medical treatment for the ill effects of addiction including medical and nonmedical treatment for opioid dependence alcohol problems outpatient withdrawal management (day detox) as well as the treatment of clients experiencing chronic pain. As a member of an interprofessional team you will provide clientcentered psychosocial support and assist clients in accessing and maintaining the social determinants of health and motivate clients to seek further treatment as well as remain abreast of appropriate resources. You will support clients to access basic necessities provide access to options for income support and support client development of life skills. You will liaise with other services at CAMH and community agencies to coordinate care for clients including linking clients to community supports liaising with their community providers and be a strong advocate to help clients gain access to important resources. You will contribute to the development of treatment plans which will involve liaising with other services community agencies and clinicians to coordinate and deliver care for clients in an efficient and safe manner. COMPASS works within a harm reduction framework: your role will include being responsible for facilitating access to harm reduction supplies. Other related duties as assigned. You will support a workplace that embraces diversity encourages teamwork and complies with all applicable regulatory and legislative requirements. This position is located at 100 Stokes Street. Some evening and/or weekend hours may be required.

The successful candidate will possess a 3 Years Community College Diploma in a healthrelated science discipline combined with 12 years of experience in communitybased case management. You must possess strong collaborative interpersonal and communication skills along with previous case management experience. Demonstrated ability to work with clients of diverse backgrounds within an addiction medicine framework is necessary. You have demonstrated ability to intervene effectively in clinical situations requiring the prevention and management of challenging behaviour. You have strong organizational skills and must be able to communicate effectively both verbally and in writing. Good computer skills are required. Bilingualism (French/English) and/or proficiency in a second language would be an asset.
