Job Description

OPEN JOB: Primary Care Physician
LOCATION:La Quinta California
***Relocation Assistance Available

SALARY: $250000 to $280000

The organization is a leading PACE (Program of AllInclusive Care for the Elderly) Provider. They help older adults (individuals 55 with complex care needs avoid nursing homes and instead receive the care and services necessary to help them besafe comfortable and healthywhile living in their own community. Their center based facilities offer Medical Care and Coordination Dental Vision Hearing Inhome Care Prescriptions Physical and Occupational Therapies Meals Social Activities and Transportation.

The Primary Care Physician performs inperson comprehensive assessments and coordinates with the team to develop a care plan for each participant.

At the direction of the Medical Director the Primary Care Physician oversees all care of participants in nursing homes and hospitals including admits and discharges for acute shortterm and longterm placement and the use of medical specialists to ensures effective care coordination.

The Primary Care Physician also provides direction and support to Nurse Practitioners and other members of the Team.

Essential Job Duties:
  • Provides assessment planning primary care and continuous medical coverage to participants
  • Collaborates with communitybased physicians and IDT in discharge planning from acute or longterm settings
  • Actively participates in IDT communicating participant changes and collaborating on care planning decisions and coordinating 24hour care delivery
  • Helps maintain care of participants in the nursing home making regular visits as dictated by participant need
  • Takes part in rotating night and weekend call

Job Requirements:
  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
  • Doctorate Degree
  • Board Certification Internal Medicine or Family Practice or Geriatrics (preferred)
  • Active California Physician License without Restrictions
  • Active state CDS Certification and DEA
  • Must be currently enrolled or eligible for enrollment to provide services to Medicare and MediCal participants
