Are you the one were looking for

We are seeking a dynamic highlymotivated Health Navigator who demonstrates our values of compassion accountability responsiveness and empowerment to our clients families and each other.


Under the Resettlement Assistance Program (RAP) the Government of Canada helps Government Assisted Refugees (GARs) when they arrive in Canada by providing direct financial support and funding immediate and essential services. The Wesley Resettlement Assistance Program located in Hamilton & Brantford ON delivers RAP immediate and critical services to clients generally within four to six weeks of arrival in Canada.


The Health Navigator will support governmentassisted refugees in enhancing their wellbeing and mental health. This role aims to help individuals manage stressors trauma and other challenges that may impact their daily functioning. The Health Navigator will also work closely with community partners and provide referrals to appropriate services as needed.


This position is a unionized fulltime permanent position. The rate of pay is $29.86 per hour. Working hours are 37.5 weekly.

Key Responsibilities:

  1. Facilitate opportunities to engage individuals in a supportive manner that will support discussion around stressors trauma and other issues that may impede healthy functioning.
  2. Provide appropriate referrals to mental health services in the community including crisis support as needed for ongoing support.
  3. Works collaboratively with RAP staff/supervisors and managers to develop implement and monitor case plans
  4. Completes administrative tasks maintains effective recording/tracking systems and responds to queries
  5. Works in partnership with professionals from other organizations that support clients such as social care health police education youthserving agencies and early childhood services
  6. Evaluate the program on an ongoing basis and recommend service delivery changes.
  7. Adhere to Wesleys policies and procedures. Attend staff meetings and training sessions as required.
  8. Provide clients with education on selfcare strategies coping mechanisms and available community health resources.
  9. Maintain required participation records and relevant program statistics to update through ICARE continuously looking for efficiencies to implement within the program.
  10. Other duties as assigned.

What you will bring:

  1. 5 years relevant practical experience with target population and B.S.W. or M.A. in Social Work or Psychology.
  2. Strong case management experience with demonstrated knowledge of best practices.
  3. Proven ability to demonstrate Wesleys mission vision and values.
  4. Ability to work in a fastpaced environment and highstress situations.
  5. Knowledge of the settlement services and integration process and working knowledge of challenges faced by newcomers to Canada.
  6. Extensive knowledge of mental health services in Hamilton & Brantford.
  7. Extensive knowledge of community programs services and resources.
  8. A fluent understanding of one of the priority languages would be preferred: Somali Swahili Amharic or Tigrinya. This includes speaking reading writing and listening.
  9. Valid Drivers License with access to a vehicle would be an asset.
  10. Nonviolent crisis intervention and 1st aid/CPR training is required.
