Developmental Pathways (DP) has an opportunity for a Case Manager Active and Coveragein our Case Management department.This position will manage caseloads in coverage to ensure support is provided aligning with rules and regulations while using appropriate delegation. The Case Manager Active and Coverage will complete any coverage tasks for individuals in coverage including required documentation related to monitoring and PMPM activities and other duties as assigned. They will ensure the effectiveness of services delivered (as authorized by the service plan) and ensure identified outcomes are met advocate for individual service needs. This position will provide support with coverage email management research tracking assignments and paperwork support as needed such as revisions CAFs DocuSign etc. Lastly they will work with the Program Manager to determine missed monitoring amongst coverage and assist in completing handoffs as it relates to coverage.
This role offers a 4day 40hour workweek with flexible scheduling options of either MondayThursday or TuesdayFriday. The role includes the flexibility to work remotely as well as the opportunity to collaborate inperson at our headquarters office in Aurora CO and/or at offsite locations.

  • Five 5 years of experience in the field of LongTerm Services and Supports; or
  • (LTSS) which includes Developmental Disabilities;
  • orSome combination of education and relevant experience appropriate to the requirements of the position
  • Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
  • Meet our driving requirements no suspensions and no more than two 2 tickets in the past three 3 years
  • Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
    Preferred
    • One 1 year of experience in developmental disabilities/longterm services and supports

    Additional Responsibilities
    • Responsible for verifying eligibility and developing monitoring and assuring implementation of the Service Plan (SP)
    • Ensureeffectiveness of services delivered (as authorized by the service plan) and ensures identified outcomes are met advocates for individual service needs
    • Providesupport with coverage email management research tracking assignments etc.
    • Serveas subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases
    • Ensure services and supports identified in the annual service plan are coordinated
    • Providecare coordination services and interventions by referring educating negotiating and mediating with the individual and external providers/agencies
    • Completeaccurate case documentation in appropriate tracking and databases; prioritizes workload and meets all required timelines
    • Completerequired documentation related to monitoring activities and other targeted case management activities as PMPM; evaluates need and effectiveness of services requested and delivered
    • Ensurethe accuracy and timely entry and distribution of required documentation to all necessary team members including but not limited to providers individuals and family members
    • Maintainknowledge of state/ federal regulations policies and procedures regarding longterm services and supports Medicaid waiver and statefunded programs and other community resources
    • Maintain a professional and ethical manner in all interactions and meets performance quality customer service and coordination standards as assigned by department leadership; maintains appropriate boundaries
