Job Description

BronxWorks helps individuals and families improve their economic and social wellbeing. From toddlers to seniors we feed shelter teach and support our neighbors to build a stronger community. We have 65 locations throughout the borough providing a variety of programs that offer assistance in the areas of family children and youth support educational services elder care homelessness mental health and workforce development. BronxWorks is hiring Case Managers for the Homeless Outreach Team. We have provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995 and more recently via the New York State Department of Health. Our outreach teams provide mobile case management services to a caseload of over 400 chronically street homeless adults with the ultimate goal of ending their homelessness.

RESPONSIBILITIES

  • Independently manage a caseload
  • Conduct comprehensive intake assessments
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals
  • Work with clients to develop goals and service plans
  • Provide referrals and direct assistance to support clients applying and/or attending appointments for: mental health services medical care substance treatment vital documents benefits legal/immigration services and permanent housing
  • Document all interactions with or on behalf of clients; monitor and document client progress toward service plan goals
  • Maintain familiarity with social services and resources available both onsite and in the community
  • Coordinate client care with other service providers; attend regular case conferences with clients and other service providers as required
  • Attend regular supervision sessions with direct supervisor
  • Drive agency vehicle to and from client locations and appointments
  • Perform additional duties as assigned

QUALIFICATIONS

  • Bachelors degree in social work or a related field of study
  • Candidate must have a valid drivers license which has been held for at least 1 year; in the last 3 years candidate must have obtained no more than 3 license violation points
  • Oneyear of case management or related work experience preferred
  • Ability to engage and build rapport with populations with high support needs
  • Strong oral and written communication skills
  • Detailoriented and resourceful with strong time management and organizational skills
  • Proficiency in Microsoft Office suite and other standard business technology
  • Bilingual/multilingual preferred but not required

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand walk or sit for long periods of time
  • Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work
  • TB Test required with the first 120 days of employmentfor Shelters and Homeless Outreach programs
