Job Description

Responsible for recruitment information and referral to resources for exoffenders. Maintaining contact with clients in the community in an effort to offer support and guidance and linking clients with needed services to promote employment treatment and housing.

Scope of Duties:

  1. Recruit clients to participate in program including visiting the county jail to interview prospective clients.
  2. Complete intake interviews with clients and secure program participation agreements.
  3. Assist clients apply for social entitlements such as GA TANF etc. when appropriate.
  4. Link homeless clients to housing.
  5. Monitor and document client progress.
  6. Assess client needs and work with client to develop goals.
  7. Update service plan with client input.
  8. Provide motivational support to clients in program and maintain support throughout participation.
  9. Schedule and facilitate meetings with clients as required.
  10. Identify and/or develop resources and link clients to needed services.
  11. Facilitate scheduled workshops/programs/groups addressing clients needs.
  12. Develop appropriate discharge plan on all assigned clients.
  13. Participate in required internal/external meetings.
  14. Complete all required reports data collection and other documentation.
  15. Interact with all clients staff and outside contacts with respect and courtesy.
  16. Maintain client and staff confidentiality in compliance with administrative policies and procedures.
  17. Attend and participate in regularly scheduled staff meetings supervisory conferences and training sessions.
  18. Other duties as assigned.

ADA Essential Functions:

  1. Occasionally required to smell stoop.
  2. Regularly required to use hands to handle or feel.
  3. Regularly required to reach.
  4. Regularly required to stand walk sit talk and hear.
  5. Regularly required to lift and move up to 10 lbs.
  6. Ability to see clearly at 20 inches or less.
  7. Ability to observe an area that can be seen up and down or to the left and right while remain fixed on a given point.
