Hospital Discharge Coordinator RN
Location: Northwest Human Services West Salem Clinic 1233 Edgewater St. NW Salem OR 97304
Job Status: Fulltime MondayFriday 8am 5pm
YOUR ROLE:
As our Hospital Discharge Coordinator RN you will support the clinical team by coordinating patient care and ensuring smooth transitions after hospital stays. This role focuses on scheduling followup appointments after hospital discharges to meet HRSA and OAR guidelines. You will track Emergency Department visits related to substance use disorders to support IET metric goals.
This position works closely with care providers and team members to ensure patients receive timely and appropriate care aligning with the PatientCentered Primary Care Home (PCPCH) model.
QUALIFICATIONS:
Current Oregon RN license
2 years RN experience
Ability to read and interpret care plans
Clinic experience or experience in discharge planning.
6 months of care/case management experience preferred
