Job Description
Prevent readmissionsguide patients through care transitions.
Key Responsibilities:
- Reconcile medications postdischarge.
- Educate patients and caregivers during transitions.
- Collaborate with physicians and nurses to coordinate followup care.
- PharmD with experience in inpatient or ambulatory care.
- Familiarity with TOC protocols and Medicare readmission reduction strategies.
- Empathydriven patientcentered care skills.
