Job Description

Position Summary:

Chemik Hospice Care LLC is seeking a compassionate organized and detailoriented Intake Coordinator to join our team. The Intake Coordinator is the first point of contact for patients families referral sources and healthcare providers. This role is responsible for managing the admission process verifying eligibility and benefits coordinating with internal departments and ensuring a smooth transition into hospice care.

Key Responsibilities:

  • Receive and process all hospice referrals in a timely and efficient manner
  • Communicate with physicians hospitals nursing homes and families to obtain accurate patient information
  • Verify insurance eligibility and obtain necessary authorizations
  • Coordinate with the clinical team to schedule assessments admissions and services
  • Maintain accurate documentation in the EMR system according to HIPAA and company standards
  • Educate patients and families about the hospice admission process and services
  • Ensure all required documentation is complete before admission
  • Follow up with referral sources to provide updates and maintain strong relationships
  • Participate in daily or weekly team meetings regarding pending admissions

Qualifications:

  • High School Diploma or equivalent (Associate or Bachelors degree preferred)
  • Minimum of 1 year experience in hospice home health or healthcare intake/admissions
  • Knowledge of hospice regulations and Medicare/insurance eligibility preferred
  • Excellent communication customer service and organizational skills
  • Proficient in Microsoft Office and EMR systems
  • Ability to handle confidential information with discretion
  • Bilingual (English/Spanish) is a plus
