The Opportunity:

Be a catalyst for wellbeing! As a key member of our team you will actively shape and implement programs that empower individuals and communities by tackling the root causes of health disparities. This role demands a passion for building bridges through referral outreach and care coordination delivering engaging health education and fostering strong community connections through proactive engagement and health promotion. Youll operate fluidly between community settings and diverse clinical environments (including medical dental pharmacy and behavioral health) making a tangible difference in peoples access to critical resources like food housing health information financial support and job training. This position requires outstanding English communication skills (written and spoken) and the ability to travel independently to various locations bringing your expertise directly to where its needed most.

As the Community Health Worker other duties will include:

Direct Client Services and Documentation

  • Respond to direct referrals from providers) for/to based on chief complaints in Epic under the Person Centered Medical Home Umbrella.

  • Coordinate services as needed by clients in collaboration with members of the care team; for complex system navigation (insured or uninsured); and accompany clients to specialty appointments. Individual and family support such as assisting with hoarding situations school Individualized Education Program (IEPs) or researching advocating and connecting for specialized situations on behalf of clients and their families.

  • Community and social support services such as school enrollment connect to external services like SNAP WIC DHC Aging & Disability Services. Home deliveries may have little client contact or contact outside of the clients home while home visits happen physically inside the clients living space to help clients with services such as requests for Provider intervention (domestic violence diabetes education school registration head start other extensive paperwork Lab results indicating client needs to start medication/go to hospital where the clinic cant get a hold of client etc..

  • Home visits are considered an essential function of the CHW position; clients are often most comfortable talking about sensitive issues supports needed and barriers to care as well as addressing health promotion in the comfort of their own home. CHWs are the only role in Primary Care currently working in the home and as such are sometimes the eyes and ears of the clinic. CHWs may need to carry items with them including their laptops for documentation paperwork and/or boxes of food or medications.

Health Education and Promotion

  • Collaborates in the planning and delivery of interactive health education classes and presentations for a variety of groups. Groups are generally inperson (such as walking groups grocery store tours cooking classes and healthy birth groups). Some online groups may be developed but are not intended to replace inperson groups. Shares culturally appropriate departmentapproved health education and information with individuals families and groups (written and verbal) and helps them to set achievable goals for improving their health.

  • Participates in the development of or referral to programs that encourage strong client participation increase clients ability to learn and apply health information such as the Prescribe a Bike and OSU Extension Nutrition and Cooking programs and positively affect their social determinants of health.

Community Engagement/Team Building

  • Develop and maintain effective working relationships with members of the care team to participate in regular team group and individual case conferences.

  • Communicate as appropriate regarding shared clients; official notes are documented in charts.

  • Develop and maintain effective working relationships with Communities/Community Based Organizations (CBOs) representing our client population (such as Acateco or Somali Mai Mai speaking communities).

Minimum Qualifications/Transferable Skills:

Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All nontraditional education training and experience will be evaluated.

Education/Training and Experience:

Equivalent to an associates degree and one 1 to four 4 years of experience in community outreach services providing health information advocacy social support and assistance in using the health care system to groups and families; and experience working with the community being served.
