Program Initiatives (PI) is part of the Primary and Community Based Care (PCBC) portfolio at Ontario Health. The PI team leads largescale complex crossprogrammatic projects including the planning and implementation of chronic disease prevention and management (e.g. diabetes and cardiovascular disease) to address quality access capacity and measurement gaps at the system level. Foundational to the PI work is the development and implementation of new models of care initiatives that focus on identifying opportunities to optimize the use of health human resources care settings and technology to reconfigure and streamline teams processes and structures.
Working in a fastpaced dynamic environment the Lead will be responsible for leading the planning development and implementation of models of care initiatives within the PCBC portfolio that support clinical quality improvement services redesign and transformation. The Lead will build strong relationships across Ontario Health as well as with key health system partners to advance priorities across different health care sectors and clinical program areas with a particular focus on integrating chronic disease initiatives with Primary Care Digital Health and existing clinical pathways.
Here is what you will be doing:
Major Responsibilities
The key responsibilities of this position are:
Project work:
Designs develops and implements new models of care particularly in chronic disease prevention and management to successfully deliver on accountabilities;
Develops a full suite of implementation supports (tools resources education material standards) to support the implementation of newly developed models of care;
Manages project workstreams and deliverables of a high priority complexity longterm duration and corporatewide scope which span various functional areas of Ontario Health;
Identifies and executes required project management standards processes and tools;
Accountable for all project management activities and documentation including gating;
Develops and presents material for review and informational purposes using excellent written verbal and graphical communications skills with the ability to express complex concepts effectively;
Guides project staff and business units in the achievement of deliverables;
Identifies improvement opportunities and escalates key issues/risks; proposes and executes solutions to address challenges or mitigate risks;
Conducts regularly scheduled status reviews provides business guidance identifies deviations to work plan and implements corrective actions;
Provides management and health system partners with status updates feedback and appropriate reporting on the project;
Leads collaboration across multiple Ontario Health portfolios emphasizing interdependence shared overall objectives and the need for combined skills with a particular focus on chronic disease initiatives ensuring alignment with the broader strategic direction;
Performs crossfunctional and/or other duties consistent with the job classification as assigned or requested.
Here is what you will need to be successful:
Education and Experience
Bachelors degree in Health Science Health Administration Business Administration or related discipline or equivalent education/experience;
Masters Degree in one of the above disciplines is an asset;
Project management certification is required;
Minimum five years of project management experience is required with a strong preference for project management services redesign and/or health systems planning experience in a healthcare environment;
Knowledge and Skills
Ability to successfully execute on largescale complex and highrisk projects or deliverables;
Experience in change management process development and business/project management operations is preferred;
Experience in chronic disease management including diabetes and cardiovascular disease prevention early detection and intervention is an asset;
Able to function independently in setting objectives for a team leading team members and managing expectations;
Superior ability to analyze vaguely defined problems evaluate alternatives find solutions and facilitate decisionmaking;
Excellent written and verbal communication with the ability to present complex concepts clearly and effectively;
Able to excel in a fastpaced and objectivebased environment;
Able to prioritize and effectively anticipate and respond to issues as they arise;
Must be teamoriented possess a positive attitude and work well with others;
Strong relationshipbuilding skills with demonstrated experience engaging and building relationships and consensus amongst diverse stakeholders;
Superior attention to detail with excellent organization and time management skills; and
Advanced knowledge of Microsoft Word PowerPoint Project Visio and Excel.
