Position Summary:
Chemik Hospice Care LLC is seeking a compassionate and experienced Director of Nursing (DON) to lead and manage our clinical operations. The DON plays a critical role in ensuring highquality patientcentered care is provided in accordance with federal state and hospice regulations. This leadership position oversees the interdisciplinary team ensures clinical compliance and helps maintain a culture of excellence and compassion.
Key Responsibilities:
- Oversee all clinical operations to ensure compliance with Medicare state and hospice regulatory standards
- Supervise and support nursing staff including RNs LVNs CNAs and other clinical team members
- Ensure the development implementation and evaluation of individualized plans of care for all hospice patients
- Monitor and ensure quality care delivery and documentation in accordance with agency policy and procedures
- Conduct regular chart reviews and quality assurance audits
- Participate in the hiring training evaluation and ongoing education of clinical staff
- Collaborate with the Medical Director and Administrator to oversee patient care and resource management
- Lead IDG (Interdisciplinary Group) meetings and participate in care planning
- Act as a liaison with patients families and referral sources to ensure satisfaction and effective communication
- Maintain a strong working knowledge of hospice regulations including Medicare Conditions of Participation
- Prepare for and participate in state and federal surveys and audits
Qualifications:
- Current and valid Registered Nurse (RN) license in the state of Texas
- Minimum of 3 years of nursing experience with at least 1 year in a hospice leadership role
- Strong knowledge of hospice philosophy practices and regulatory requirements
- Excellent leadership communication and organizational skills
- Proficient in EMR systems (e.g. Alora WellSky Kinnser) and Microsoft Office
- Ability to lead coach and motivate interdisciplinary teams
- Strong critical thinking and problemsolving abilities
- Current CPR certification
- Valid drivers license and reliable transportation
