COMMUNITY HEALTH WORKER

Red lake homeless sheltercommunity health service program

Open: April 2 2025 Until filled.

Position Summary:

The Red Lake Homeless Shelters Homeless to Homes Programs is a Housing Support program developed to address multiple barriers facing individuals who are currently housed/unhoused. The Community Health Worker (CHW) will work with the vulnerable population in providing education and support for healthrelated topics that may have historically created barriers for individuals who are homeless or at risk of homelessness. The position will require working in an office setting as well as making home visits to individuals participating in the program. The position may require working outside traditional office hours to possibly include weekends and holidays. The person in this position must be able to work with individuals with a wide range of backgrounds and demographics. Majority of time will be spent meeting with clients and effectively managing time. Reports to Executive Director/ Red Lake Community Health Services Program Director Full time position w/benefits. Salary; DOQ.

ESSENTIAL RESPONSIBILITIES/DUTIES:

This position will be responsible for working under the supervision of a Medical Assistance enrolled physician registered nurse advanced practice registered nurse or mental health professional and will be responsible for the coordination of the initial visit to establish Community Health Worker services. The position will also be responsible for:

  • Provide communitybased services to eligible adult clients (age 18 with health conditions that impacts their ability to live successfully in the community.

  • Assist individuals with the development of health goals and plans.

  • Coordinate and refer individuals for appropriate medical needs and attend appointments upon request.

  • Develop educational health related materials and curriculum to

  • Document services and visits in appropriate electronic health records system.

  • Communicate important information pertinent to the clients Housing Case Manager and other professionals working with the client.

  • Schedule and attend appointments with participants in program.

  • Motive individuals to be active engaged and participants in their health.

  • Coach patients in effective management of their chronic health conditions and selfcare.

  • Meet with individuals 25 hours a week in the community shelter or in the individuals home. It is expected that 20 30 hours a week will be spent in direct client interaction with a full caseload.
  • Ensure that services are delivered in a manner that is culturally sensitive and appropriate following a holistic and personcentered delivery method.
  • Support the person in applying for all benefits (county state tribal and federal) that will help them afford their housing.
  • Participate in supervision under the direction of designated supervisor.
  • Provide program and client transportation as needed.
  • Maintain a caseload of at least ten clients or more as assigned meeting productivity guidelines and proactively seeking referrals as needed to maintain a full case load. Typical caseload will be 15 clients.
  • Maintain billing and documentation in accordance with Department of Human Services guidelines and submit billing according to procedures established by department: daily progress/encounter notes billing encounters monthly progress reports and file reviews in health records (either paper or electronic).
  • Input data into the Procentive system in a timely manner for reporting and funding purposes.
  • Provide client level data for periodic (quarterly every 6 months and annual) reporting for grant funded programs.
  • Participate and assist in the tasks associated with client care including evaluation of client safety and appropriate interventions related to their functioning and safety.
  • Acts as an advocate for clients as needed.
  • Provide crisis assistance as needed.
  • Attend trainings and conferences to comply with all continuing education requirements and further enhance technical and programmatic knowledge.
  • Abide by and comply with the rules of confidentiality Data Privacy Act mandatory reporting laws HIPPA requirements and organizational policies and procedures.
  • Attend community meetings as requested by the Executive Director. Performs other duties as assigned.
