Description

Discharge Planning Coordinator / RN

Oncology

Shift: Full Time 7:00 am 5:30 pm

Location: Emory University Hospital

  • Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process.
  • Promotes the safety and well being of all patients by coordinating the patients discharge from the hospital and transition of care to the outpatient clinic.
  • Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care.
  • Works in collaboration with others to ensure that all discharge needs are met. Schedules all discharge appointments treatments and protocol specific orders.
  • Completes needed discharge follow up phone calls to patients and documents appropriately.
  • Performs other duties as required.
  • Works with manager to formulate plan for professional development.
  • Attends educational inservices as appropriate.
  • Participates in professional activities and organizations to maintain knowledge of current trends practices and developments.

Minimum Qualifications:

  • Bachelors degree in Nursing.
  • Must have a valid active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
  • Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration.
  • OCN certification preferred for some positions.
  • BLS required; ACLS Certification preferred but may be required in some areas.
